Miller-Dwan Foundation Unrestricted Fund

Eligible entities include:
Miller-Dwan Medical Center
Community Nonprofits

Programs and services must fit within the grant-making interests and historical specialty areas of the Foundation. Projects may fall along a spectrum from prevention activities, to assistance within the medical community, to community-based programs.

Van Gorden New Initiatives Outreach Fund

Eligible entities include:
501(c) 3 non-profit organizations in our geographic focus area

This fund is designed to support and promote innovative new projects related to physical rehabilitation services, research and education, as well as the promotion of creativity and excellence in rehabilitation services programming.

Van Gorden New Initiatives Polinsky Fund

Eligible entities include:
SMDC/Polinsky Medical Rehabilitation Center projects/programs

This fund is designed to support and promote innovative new projects related to physical rehabilitation services, research and education at Polinsky Medical Rehabilitation Center, as well as the promotion of creativity and excellence in rehabilitation services programming.

Sara M. Young Education Fund

Eligible entities include:
Polinsky Rehabilitation Center and Miller-Dwan Medical Rehabilitation Center.

The Sara M. Young Fund was established in honor of Sara Young, a supporter of Polinsky, physical rehabilitation services and the needs of people with disabilities. This fund is designed to provide educational opportunities to rehabilitation patients, family members and practitioners. Consideration is given to projects providing effective and innovative education programming, library resources, rehabilitation programming, workshops, seminars and educational materials.

Albert G. Gonska Fund

Eligible entities include:
SMDC/Polinsky Medical Rehabilitation Center projects/programs.

This fund is comprised primarily of funds donated by Albert Gonska, a compassionate community supporter of programs for children with disabilities. It is designed to assist in the provision of high quality, innovative rehabilitation services and programs for children.

Dorothy Ribenack Fund

Eligible entities include:
Polinsky Medical Rehabilitation Center and the Duluth Clinic Fitness and Therapy Center.

The Dorothy Ribenack Fund supports and promotes new and innovative rehabilitation service projects that foster creativity and excellence including research, education and facility improvements.

Abilities First Fund

Eligible entities include:
Individuals with physical disabilities and limited financial resources who reside in our geographic service area.  Eligible applicants are restricted to those individuals’ who have a primary diagnosis of spinal cord injury, stroke, brain injury, congenital disability, or other documented neuromuscular or neurological trauma or disorder.

The Abilities First Fund purchases physical rehabilitation equipment and aids that allow those with physical limitations to live life more fully and integrate in the community, workplace, or school.

Designated Funds

Eligible entities include:
Internal departments focusing on the historical specialty areas of Miller-Dwan and Polinsky; and Solvay Hospice House

Designated Funds are used for specific health areas and have been created through the generosity of numerous community individuals and employees.  Funds are used for purchases or projects that assist in patient care and comfort, including new equipment, environmental improvements, patient and family education and staff education.
